I think "Spilaiotis" is not his last name, but means "Cave-dweller" as he is also known as. This means that his parents do not have this as a last name. - If Spilaiotis means Cave-dweller, then it should definitely be removed - from his parents' names, the intro and the title.

I have seen icons and books of Elder Joseph referred to as "the Hesychast" as well as "the Cavedweller" (or Spilaiotis in Greek) so the title and intro can show that.
