New template for Copyright issues/deleted images

One or more of the images you recently uploaded did not contain enough information for us to verify that it would be legal for us to use here. It has been (or will soon be) deleted. If you can verify that we would have permission to use it here, preferably under our [[OrthodoxWiki:Copyrights|default license arrangement]] (or a [http://creativecommons.org Creative Commons] license, unless it is in the public domain), we would be happy to restore it (or you can simply upload it again). We are grateful for your work here, but just need to be careful in order to free ourselves from legal complications down the road.
