Clarification on this Sts commemoration date

Umm, Im truly confused. There are two commemoration dates for this one saint and different sources state a different date ... Can someone assist on the authentic Orthodox churches position on this saint rather than pulling it off Internet sources (even these can be in error since it is people like you and me that put them together).
